Kerry Cartier and his team of Chapter Safety Net Representatives
have developed a reporting form which should be sent to Kerry whenever
any rotorcraft incident, no matter how slight, occurs. This will be completely
anonymous and will be used to find overall causes of gyro incidents – not
to place blame on a particular make of gyro but to find possible correlations
between certain aspects of all rotorcraft and incidents. For example, there is no
concrete research about horizontal stabilizers and their relation to gyro
incidents/accidents, only anecdotal reports. NTSB reports do not help since
most rotorcraft incidents are not reported. An earlier rendition of this form appeared in the
April, 2001, issue of the PRA Rotorcraft magazine. The newest version is now also
downloadable here from the Internet in IBM and MAC formats: Word, WordPerfect and Works for the IBM computers
and two MAC formats. Older Microsoft Word programs (such as the one used with Win
3.1) can use one of the MAC formats which will be converted correctly. The
downloaded file should be saved on your hard drive, renamed (or copied) and then, using
overstrike/typeover, you can write on the form, save it, and send it as an
attachment via e-mail to Kerry Cartier. Or you can mail it to him -- he does
prefer the mail since he can easily confirm the authenticity of the sender. Or
you can simply fill in the on-line form which goes to him directly and not to
anyone else!

Six/Seven Different Formats If you have an IBM compatible computer and Microsoft Word from Office
97 or later then right click on the file name and download
the file (usually your choice will be "save link as" for Netscape and "save
target as" for Internet Explorer): rotorinc.doc
If you have an IBM compatible computer and Corel WordPerfect
or even an older WordPerfect for Windows 3.1 then right click
on the file name and download the file (usually your choice will
be "save link as" for Netscape and "save target as" for Internet Explorer)

If you have an IBM compatible computer and earlier version of Word
(such as Word 6.0) for the IBM then right click on the file
name (usually your choice will be "save link as" for Netscape and "save

name (usually your choice will be "save link as" for Netscape and "save
